Sharing a case of multiple coronally advanced flap with Emdogain application to improve the gingival recession from LR6-LL6 following 2 separate FGG surgeries (LR1-LR6 and LL1-LL6).

The primary challenges in this case here are the complete absence of keratinised tissue and the buccal/labial displacement of teeth from the alveolar bone.

In an ideal world I would love all these teeth to be tucked in slightly, however she was referred to see me following completion of orthodontic treatment by an orthodontist following Class III camouflage. That was the best we can get!

Sharing a root coverage case to help improving the occurrence of gingival recession in a patient following a history of 2 courses of orthodontic treatment.

In an ideal world, we’d love for all recession cases to be completely predictable but that’s not always the reality. Setting realistic expectations and clear goals with the patient before surgery is key.

In this case, the result was a success, as it matched what we aimed for with the RT2 type gingival recessions. Other success parameters to consider would be reduced discomfort in tooth brushing and dentin hypersensitivity. Definitely would wish for more improvement on UR1 and UL1!

After 18 months the outcomes were still very stable.

Saving one tooth at a time!

This patient presented with generalised periodontitis stage 3, grade C according to the EFP classification.
The LR4 was associated with an extensive circumferential bone loss associated with mobility grade 2.
Following non-surgical periodontal therapy, splinting of LR4 was prior to periodontal regeneration.

Intra-surgically the bone defect was extended to the apical third, it was rather contained in the lingual aspect except at the mesiobuccal aspect. There was a consideration for GTR however, because of the thin gingival phenotype, a combination of enamel matrix derivates/Emdogain @straumannglobal and xenograft @geistlichdental was done.

Overall the outcomes achieved at 1 year were really good with mobility improved to grade 1 but not perfect with residual 5mm localised sites. The splint was removed after 1-year. Continuous NSPT was provided for the patient every 3monthly, and the outcomes achieved was stable at 2-year

In hindsight, I should have considered splinting the LR4 earlier during the NSPT phase. Wire splint will be easier to remove but patient did not want it due to aesthetic concerns.

I would like to share this case, and I think there

I would like to share this case, and I think there are some learning points to it. This patient lost the UL1 due to internal root resorption with continuing endodontic infection, as a result, she lost a significant amount of bone both horizontally and vertically. Subsequently, the UL1 was extracted followed by alveolar ridge reconstruction. 5 months later the implant was placed with further vertical augmentation. Following 6 months of healing, I took over the case at the baseline situation to restore the implant, which I have to say is much easier than what my colleague has done, and she did such a wonderful job with the implant placement. In this case, we opted for wax-up 1 to reduce the restorative need and maintenance.

I think the overall aesthetic outcomes are quite pleasant, although not perfect. I would have liked the gingival margin level to be equal to UR1. However, considering the amount of vertical loss we had from the beginning, we knew this would not come back. I wish I had the pictures but the final gingival margin level of UL1 was comparable to the initial situation which was more apical than the UR1.

One of the biggest learning points from this case is the loss of gingival pigmentation. At the baseline situation, we can appreciate some loss of gingival pigmentation due to GBR but this became more apparent following the second stage surgery. Fortunately, the patient’s smile line was moderate and the UL1 gingival margin was not showing on smile.

Did some digging and saw a few case reports that reported on the lost of gingival pigmentation following GBR, and some of the solutions used were pigmented free gingival graft harvested from buccal attached gingiva of posterior teeth/ pedicle flap of adjacent pigmented portion of buccal attached gingival. In this case, nothing was done as it was not bothering the patient.

Therefore, I think it is important to consent for this potential aesthetic complication with patients who have gingival pigmentation. Checking the smile line would also help to recognise the potential problem and manage it accordingly.

Protruded upper front teeth

Significant protrusion can be a big problem as it increases the risk of injuries to the front teeth and often causes lip incompetence — the inability to close the lips properly — which can lead to dry lips and a higher risk of gum problems.

In this case, the patient had a history of trauma to the front tooth before treatment. The tooth was slightly loose but still vital.

She was successfully treated with a functional appliance and metal braces, without the need for any extractions.

Impacted teeth.
We had a lovely time treating this patient. She came to us with a few problems:
- impacted canine
- impacted premolar
- supernumerary tooth
- crowded arches
- dental midline shift
The impacted canine had caused some degree of root shortening to the neighboring lateral incisor, so we had to do something urgently.

One of the problems of leaving an impacted tooth in the bone is the risk of it damaging the roots of the neighboring teeth. Cysts can also form around the impacted tooth.

Therefore, although it is not always necessary to intervene in the case of an impacted tooth, if we’ve decided not to do anything, regular monitoring of the impacted tooth for root resorption and cystic changes is required.

This charming young man came complaining that he was unable to chew with his front teeth. He had an anterior open bite which means there was a gap between his upper and lower front teeth. He also had severe crowding of both arches with an impacted canine, and posterior crossbites.

He had upper and lower fixed metal braces with removal of 4 teeth. Treatment took 27 months and he was rewarded with a very lovely smile at the end!

She has a condition called molar-incisor hypomineralization (MIH) which means that the quality of some of her molars and incisors are not that great. This was the cause of her tooth sensitivity.

Her treatment involved extractions of 4 teeth, including 2 first molars which were of poor prognosis.

Treatment took 30 months, with most part of it spent closing the molar spaces. Molars are huge teeth! She ended up with a very lovely result and best of all, we got rid of her tooth sensitivity!
